FOR A DOCTOR/SPECIALIST

 Directory for all Doctors and/or Specialists:
FrontPath Network Doctor and Specialist Directory

This is websites does not require a  “log-in” or any submission of information.  The FrontPath Health Coalition is the network directory for the City Insurance program.  All employees are covered by this network, with the exception of Firefighters and Teamsters who are covered on different plans through their bargaining agreements.  This directory will provide information on family practice doctors and specialists of all kinds, in the regional area that the City of Toledo will cover.

Central Benefits provides the City with the Administration of the medical plan for City of Toledo employees and their families.  When you visit their website, and log-in using your information provided by CenBen.

FOR A DENTIST OR TO FIND INFORMATION ABOUT YOUR DENTAL BENEFITS:

Directory for all Dentists:
Delta Dental Directory

This website does not require “log-in” or submission of personal information beyond your location.  All employees are covered by this network, with the exception of Firefighters and Teamsters who are covered on different plans through their bargaining agreements.

The Delta Dental Plan of Ohio company provides the City with three discount networks, Delta Dental Premier, Delta Dental PPO, and DeltaCare USA.  While City employees and their dependents can go to any dentist in any of these networks, the deepest discounts—for both the employee and the City—are the dentists in the Delta Dental Premier network.

**This website does not provide information the benefits covered by the City of Toledo.  Please review the appropriate plan document to determine what services are provided to our employees.

Directory for Vision Providers:
Find a Vision Network Doctor

This website is for EXEMPT employees only.

It requires a “log-in,” that is set up on the first visit, and can be done by both, member or dependent spouse. The Superior Vision company provides the City with a discount on glasses, office visits, contacts, etc.

This website provides information to the registered member on eligibility and services, as well as doctors/providers.  It is a secure site that does require a member ID and other information for verification.
