TUESDAY, NOVEMBER 24, 2009 UNTIL 8:00 A.M. MONDAY, NOVEMBER 30, 2009. CONTRACTORS MAY FAX (419 245-1329) THEIR PERMIT APPLICATIONS WITH A CREDIT CARD AUTHORIZATION FORM IF THERE IS NO PLAN REVIEW IS REQUIRED.
